Anaesthetic Rooms

Hospital Registration Form
The forms in the Hospital pack MUST be completed and returned to the Hospital at least one (1) week prior to your surgery. Our hospitals all have online registration availability if you prefer.
Consent Form
At the time of your consultation your surgeon will ask you to sign the Acknowledgement of Information form and the Consent form .
Instructions
PLEASE DO NOT EAT OR DRINK ANY FLUIDS FOR SIX (6) HOURS PRIOR TO THE PROCEDURE.
Surgeon’s Fee
The Surgeon’s fee which is the quote/invoice given to you at the time of your consultation is to be paid through our Consulting Rooms prior to your surgery
